My work in sexual dysfunctions is exclusive in the entire region  and I am the only Private Practicing Psychiatrist in Chandigarh and in the surrounding states of Chandigarh who is well equipped technically in dealing with Sexual Dysfunctions .

The general outlook of common cases I routinely deal with are mentioned under. I will start with the simpler ones to the complex ones. The field is vast still I will try to cover the most common ones —

MALE SEXUAL DYSFUNCTIONS

IMPOTENCE

Its called asdifficulty in getting and sustaining erection. The intensity of complaints vary and depending upon that people suffering from Impotence come with following type of questions---   

• I have partial erections ie the penis is not fully erect,
• loosing erections during intercourse,
• able to have erection while masturbation but not able to have erection with wife,  sometimes the things are different …a person is able to perform with person outside wedlock but not with wife.

The inability to perform successful intercourse, the patients present with

• unconsummated marriages,
• stressed/unsatisfactory  married life
• undergo /threatened Divorce litigations 
• Mental depression, [which is sometimes both in the patient and the spouse.]
• Inability to have children.

PREMATURE EJACULATION

“In simple words its getting discharged too early within few seconds or even outside, or before reaching climax”

The patients usually come with complaints directly that----

• He comes too early
• His partner remains unsatisfied and he too does not enjoy so small session of sexual intercourse.
• He feel loosing his self image and self confidence due to it….some develop depressive syndrome too.
• The sufferer looses interest in making sexual contact with his mate.
• Due to premature ejaculation, the patients later on start suffering from Impotence too, because the desire to make physical contact reduces. ..NO AROUSAL à NO ERECTION àIMPOTENCE

DELAYED EJACULATION 

Its the failure to reach climax or inability to ejaculate ,or ejaculate after too long and getting fatigued by it.

This disorder causes lot of discomfort to both the partners due to following reasons.----

• The male member has to pump on and on in a fit to ejaculate, he becomes physically fatigued and tired by just pumping mechanically into his partner.
• As I mentoned male member does not reaches climax., so he many times have to stop without ejaculating.
• He goes on moving in and out mechanically, without sensing much pleasure.
• The female partner start feeling pain and discomfort from senseless thrusting.Male also feels a ache in penis or rashes or cuts in prepuce
• Soon the sex becomes dry due to lack of or drying down of sexual secretions, as there is no excitement in the sexual organs of both the partners
• The Sexual act seems like a burden to both partners so there is less frequency of contacts and sometimes loss of warmth between the partners too.
• The male sometimes prefers to masturbate to ejaculate and get pleasure by himself.
• Female start feeling………she is not that attractive now to her husband and feels inferior /incomplete/ less feminine in herself.

FEMALE SEXUAL DYSFUNCTIONS 

There are following types of female sexual dysfunctions.I am mentioning them briefly--

Frigidity  it’s the  inability to achieve sexual pleasure

Sexual pain disorders…Pain during intercourse.

Hypoactive sexual disorder-Decreased interest in sexual contact and had a attention to avoid it.

Vaginismus--its a psychological condition where a women in unable to relax his vagina  and does not allow the penis to enter her…….its easily treatable too.

Ferafulness of intercourse to avoid the pain of delivery which occurs as pregnancy matures

HOMOSEXUALITY ISSUES

Non inclination of the grown ups towards marriage is he/ she homosexual? 

Is homosexuality treatable?

Should homosexuality be treated or accepted?

Is it really a mental disorder ?

Homosexuals parents worried about their young adult,s same sex  inclination

Parents bringing their grown ups for treatment of homosexuality

SEX EDUCATION –

Patients both adults and teenagers  come to consult me to get educated about the day to day problems they are facing  or to get the scientific answers for some questions which disturbs them or according to them which is a problem or can result  into a sexual problem later on..

Sex education is delivered to patients coming with following types of querries. Some of the commonest reasons for which patients seek information are ---

Masturbation/ Self Stimulation…

This is a favourite topic equally with teenage boys as well as adult males. The common questions which are asked by my patients are---

• is it harmful
• it is normal
• How much to be done
• Does it lead in physical wekness……
• Does it decreases learning capacity and affects concentration
• Does it lead to weakness of eyes
• What age it has to be started
• What to do if I come to know my son is masturbating
• Does it lead to sexual incompetency in married life
• Does it weakens the penis……..weakens the nerves…..
• Does it affects the penis size
• Can a married person masturbate

Wet Dreams/ Night Falls

 Ejaculating while one is in sleep at night/ discharging the semen in dreams. 

The fact is that its not a disese and can be prevent by simple instructions and educating a person about his sexual system. But its posed as an illness by road side or self styled sex specialists. The rather harm deeply the psych of the patient

The patients usually ask--

How can I cure them ..its big problem

I feel too guilty when I had night fall and my clothes get soiled

Its making me weak………..what should I do for it

Night falls are coming too frequently…….

They last for several days

I use to have lot of sexual dreams…..which lead to discharge.

What to explain to my son when I see his pants having a discharge

I have taken lot of medicines for it from so many doctore but its not cured.

DHAT [One of culture bound Syndromes in India.]

Oozing out of semen /whitish substance/ colourless fluid while passing stools  in  the urine.

This is a very interesting phenomenon and is culture bound in India and south east asian countries and is populatr by different names.

Again its not a sexual disorder at all but is popular as “so called”one of the common sexual disorder.It has roots down in Indian culture and interestingly according to the Ayurvedic Indian system it’s a very serious disorder as its portrayed to be extremely harmful to loose ones “precious semen”

There is a very strange theory put behind it by ayurvedas…..Very interesting indeed to know…….Its somewhat in this way

” one drop of semen is  made of 100 drops of blood, and one drop of blood is made by so much minerals and nutrition taken by the male. It takes lot of time for semen to reform again and become mature enough to produce ”

So loosing even one drop of semen results in--

       extreme weakness,

• failure of a male to become father as semen is lost,
• it leads to weight looss,
• black circles under eyes,
• weakening of eye sight,
• the semen becomes watery,
• it becomes less in quantity….

and what not..rest you can imagine , a person reading all this , what a picture he will form about himself.

In all its enough to horrify a male person and he started feeling like he is criminal and that he is useless, unfit for marriage.

Its not all here…” Masturbation and Dhat Syndrome are linked with each other ie the person who masturbates also start having Dhat syndrome and thus heading into a vicious trap of disorders….    

This all  can be prevented by providing the patient/ victim a few sessions of Sex Education with a goal to educate him about this phenomenon and the his sexual system. But as mentioned before its portrayed as an illness by self styled sex specialists/quacks in India. They charge heavily a person for curing this “so called severe degree of sexual Dysfunction” after terrifying him to his roots.

Or it may not made my teen angry……….or it may noy make my kid think  that I am thinking so low ……….how can even I think like this about them
